Make form for users to help requests #13

Is your feature request related to a problem? Please describe. Currently the button to report requests is not working.

Describe the solution you'd like Show a modal / bottom sheet with a form that contains step-by-step wizard where user fills all required data to report a person with symptoms. The user could be reporting this for themselves or for some other person.
