General clean-up #31

This PR organizes files into folders, rewrites install instructions in README.md, updates metadata in the .theme.css & .user.css files, and officially removes Powercord and Replugged support. Also bumped version number from 2.0.2 → 2.1.0. Pretty sure all links have been updated to follow the new folder setup, but may have missed one. If so, just let me know and I'll find & fix it.

is there any reason why removing replugged/powercord support? also fix the conflict since i already merged PR #30

Yep! From what I can tell, Powercord has been dead for over a year now [latest commit] and Replugged just isn't working anymore. If you go to the link currently in our repo, you can see it says that we're out of date for them. I think that this theme was built for their legacy version, and is no longer compatible as mentioned in their README.md here.

ahh, that make sense. just checked that replugged dropped powercord support. anyway lgtm!
