Converted index.htm to html5 #3

  1. Changed name index.htm -> index.html.
  2. Moved out styles and scripts to files.
  3. Changed doctype.
  4. Moved scripts declaration to end of page for spead up page loading.

Thanks! Lots more tidying to do, but this is a good start.
