Adds end-page column to remaining of "C" instrument books that I have #27

As mentioned in https://github.com/aspiers/book-indices/issues/26#issuecomment-656584449, my PDFexploder software doesn't need these, but they are valuable anyway, so I'm merging. Thanks!
