Add index for The Slick Book 1 #36

The reason I added the book name was that you can map that field to keywords in forescore, which makes it easier to navigate songs by their book without having to find the right file name to open (since there is no way to list files and bookmarks separately).

I see. Does it have to be the second field (column), or could it be a later one after the page start/end fields we already have?

Forescore allows any column order, since the user needs to map each column to a field. So I could rearrange the columns to have title, start, end, artist/composer, keywords. Ok?

Also, what do you think about including column headers? This would make it easier to understand the meaning of the columns. Forescore supports this, but I’m not sure about other applications.
