Update migrate-sql-server.md #1062

Removed product links in favor of all docs links, changed wording slightly, and moved Get Started to be at the top.

Cam Soper ported all of these to docs.microsoft.com back in November, so at this point these should be considered dead and you should work with him to update the official doc at https://docs.microsoft.com/en-us/dotnet/azure/dotnet-howto-migrate-sql?view=azure-dotnet
