Refactor kytos sdx topology

Some changes on this PR:
Moving files from the
app
subdirectory to the root
adding
CHANGELOG, LICENSE, MANIFEST
files and refactoring setup script
removing topology_wrong.py and topology_mock.py which are not necessary (they were only created for tests)
removing API calls that were only used for
adding requirements file
moving KYTOS_TOPOLOGY_URL to settings
ignore post to SDXLC if no SDXLC URL is provided
