Mapping using combine mapping type UX #99

I think we should support users using mapping combine type by selecting input fields and the single output field multiple times.

For instance if I have FirstName and LastName fields in the input mapping, I should be able to map those to a single field by selecting them from the input and mapping them to the single output field.

The input fields should end up being in the same mapping, currently the end up in two different mappings.
