Doc updates for 1.0.0 release #243

Before we release 1.0 of @atomist/sdm and friends, we want the docs to be ... better. This is not an attempt to make them perfect before the release, but better.

We want them to emphasize things that people want to do, to make those easy to find. Link to the necessary concepts.
