I don't understand your question. Your ODT has empty input field. You must name
it.

Ok, i will explain in detail
I want to generate a odt form in that (for eg: name and age) fields are there,
By using $name and $age i want to fill the form. But my problem is i was used
the following hyperlink approach to create input filed

But when i click on Ok buttton in Input Field dialog box $name is not visiable
in the form in that place "Input field" is found.. See the bellow attached

You should try to test if your oppenoffice can create input field. The macro
uses the OpenOffice API to create input field.

If I understand you need html->docx converter. If it's that XDocReport doesn't
provide this converter kind (html text styling for docx do that, but it works
with an existing docx).
Next time, I suggest you to create a new issue, because it's out of scope of
the original topic.
