Getting "Password not Valid" text in Moodle - is this related to our SAML?

What steps will reproduce the problem?
Logging into our Moodle server will sometimes cause this to happen.  It is 
worst at the end of our school days.  We used to be able to get rid of this by 
clearing the cache on the browsers.  Safari and Firefox both do it.

What is the expected output? What do you see instead?
Expected output should be users getting logged into Moodle cleanly instead they 
get "Password not Valid" text (see attached)

What version of the product are you using? On what operating system?
Inital Release of the Moodle Googel Integration
Moodle 1.9.9+ (Build: 20100707)
OSX server 10.5.8

Please provide any additional information below.
Starting this school year (2011-12) my users have been randomly (but more 
frequent than I would like) getting the attached error.  It states "Password 
Not Valid" and this is when attempting to log into Moodle.  We are doing LDAP 
authentication to our PDC and then using the Moodle-Google plugin for SSO.  We 
are using the first version of the plug in.
Does anyone think that my issue is my Moodle server (or should I consider 
upgrading my plug in?  Any thoughts appreciated.

We get this all the time... Cleaning out our cookies will fix the problem. Very 
annoying for our staff and students. We have the latest version of the plugin 
(4).

Josh,
Funny enough it stopped when I changed the front page theme?  Maybe permissions 
issue with graphics/objects that were in the other theme, but clearly changing 
to a different theme cleared this up.  Totally strange.
