ModuleStateChangeNotifier should use TransitionResult, not ModuleStateChange #257

Applies as of r4732

Note that TransitionResult contains ModuleStateChange, so that this change
is only a relatively trivial refactoring.

Backward incompatible, but unlikely in practice to cause problems
