broadcast: use health notification for timeouts #17

If there's a state timeout e.g. in transition from live to slate or vice versa, or starting and stopping hardware, it normally means something very wrong has happened, so maybe we should consider a health notification.

Per Issue #18, consider using a notification message kind ofbroadcast-timeout to differentiate it from other broadcast health issues.
