Make accessibility service package list configurable #175

The accessibility service has a specific array of packages it responds to.  
This list should be user configurable based on installed packages.  Right now 
(in version 2.10.2) there is a fixed list of packages.

Make a UI that lists all installed packages.  Let the user select each package 
they want to hear notifications from.

Completed in 2.11.3.  There is one problem though.  When removing packages 
(apps) from the checked list the change won't take effect until you go to 
accessibility settings and disable then re-enable A2DP Volume.  I have not yet 
found a way around this.
