Replace issue templates with issue forms [SDK-4169] #71

Changes

This PR replaces the Markdown-based issue templates with issue forms. These forms are a direct conversion of the Markdown issue templates, with the addition of a checklist at the top containing links to self-serve resources.

Sorry, I know I approved but realised after reviewing the one on auth0.js that we're not including all the questions from the original bug report - any particular reason?

In this case, the one about "Which framework are you using" is not a great one, but please replace it with one that asks about the browser being tested, similar to my comment here.

Sure thing, added in https://github.com/auth0/angular-lock/pull/71/commits/51ae892d0fd7a3a668780dd6ff6ddf07bc71ad3a (I used the browsers in https://gs.statcounter.com/browser-market-share/desktop/worldwide). Copy-paste mishap, because this field is not present in another browser library I did previously: https://github.com/auth0/lock/blob/master/.github/ISSUE_TEMPLATE/report_a_bug.md#environment.
