Add example config file #54

This PR adds an example config with rules for creating namespaces and pods.

This also fixes a couple of small things:

/openapi is allowed to go through by default
only one filter is allowed per rule; this can change in the future but the current implementation requires it anyway
switches to a single replica for dev, since we use embedded spicedb
detects one type of unrecoverable error instead of retrying it forever, but filed #53 to address this more generally
