One back/fw history button instead of two #71

In a bit of a Fx3-like way, supporting a single button for back/forward
navigation with the 10 nearest history items would be great, instead of
having a "back" history and a "forward" history.

I disagree; especially when I'm wikihopping I like having separate "back" and 
"forward" lists. I can always get the 
unified list from the History menu.

Updated patch.  Less code and no longer requires a restart.  The menu can be 
enabled under the 
Appearances tab in the options dialog.
