When you create the fields in your WIT form, do you have to input the all the values in those lists or can they simply be defined in the CSV file?

I have setup the control and fields. I cannot seem to upload the csv file using the menu in the work item type. It simply does nothing. I doesn’t give me a prompt as in the instructions.
