You dont need a picklist, you can use simple field as line string.

I created a CSV file UCM Symptom Test.csvhttps://github.com/avih75/PickerFieldsControl/files/7164021/UCM.Symptom.Test.csv with an identical name of the control, but I do not see any multi-level fields appear in the dialog box. When you create a control, you must also add a plist item in order to save. I repeated this with creating the control after the upload but still does not work.
