Water Quality Testing Form is to be created under the same existing 'Ward' subject.

Visit Scheduling Logic

1. Form should get scheduled to either May 1 or Oct 1 whichever is next. If the form is scheduled for May then it should be Overdue after June 30. If the form is scheduled for Oct then it should be Overdue after Nov 30. For Registration, within May or June, it should schedule for the current date and overdue by June 30. For Registration, within Oct or Nov, it should schedule on the current date and overdue by Nov 30.

1. Remove Pravesh ki tareeq (we added date of entry for clarity yesterday) it has been added twice, the second time as Pravesh ki tareeq, that needs to be removed.

2. Unique date logic for date ( that it should be different from date of entry) needs to be removed. We mentioned that to convey that there are 3 different date pickers that need to be on the form, not that they need to be unique dates between the 3 fields)

3. Similar to other cards on homepage, cards related to Quality Testing need to be added. Title Left card Right card
