Context

Dawa Prapatra is a document that ASHA has to submit to claim incentives for the work that she has done in the month. There are activity code related to each activity and associated rate. She has to provide the following key information

We would be creating a report that generates this and can be downloaded in a spreadsheet.

In the phase 1, we would only be covering the activity code mentioned in the ACs. The logic is explained in the linked sheet https://docs.google.com/spreadsheets/d/14vjke5eR8-afiRXKUQalxDk0E12ktltVLt1Zq_rq3AI/edit#gid=0

Acceptance Criteria

  1. Checklist of activity codes to be covered

    • [x] C 1.1 : This code is related to completing 4 ANC visits
    • [x] C1.3 : This code is related to 180 Iron Folic Acid Table issued
    • [x] C1.5 : This code is related to two TD dose or one booster dose given
    • [x] C1.8 : This code is related to PMSMA done
    • [x] C1.9 : This code is related to 7 ANC checkups completed for HRP cases
    • [x] PC1.1 : Maintaining and updating the entries

  2. Only rows where there are values should be shown

  3. Report should have an ability to choose a month and year for which Dawa Prapatra is to be generated

  4. Name of the Report : Dawa Prapatra

  5. Fields metioned in the context above should be columns of the report
