After 3.39 release, remove the dummy encounter type created #56

Once the fix for this: https://app.zenhub.com/workspaces/avni-product-635b73bded85e50018871ae6/issues/gh/avniproject/avni-client/954 is released, delete the "Test Visit don't use" encounter type, associated forms and form mappings.

@sachsk considering the related card is done, now the encounter type can be deleted
