[JSSCP] Disease Prevalance report #76

Goal Create aggregate report and linked line list for user to see following things:

Prevalence of a specific disease per 1000 population
Ratio of diagnosis by gender
Death ratio for the number of patients dying from a disease against the total number of patients for the disease

Acceptance criteria

@utkarshavni Need to confirm following:

Whether Incidence rate should be in percentage
Whether Mortality and Prevalence both indicator needed in same report and if Yes, how do we apply the date filter
Whether linelist is required
