[ ] Please pick this card only after the below changes in the ANC forms are completed which are available in the below cards:

[x] This for is already there in JNPCT. Need to add the new changes mentioned in the above card are implemented in this unplanned ANC form as well.
[x] This form will be visible for all users (Saheli, Supervisor and Cluster In charge). Need to enable the permissions for these users
[x] Add encounter eligibility Rule: It should be allowed to fill till the delivery form is filled** under Pregancy Program
