Schedule SCD followup visits for existing individuals

• [x] Write an insert script that should schedule 'SCD followup Jan 2024' for all of those individuals who
  1. Don't have any encounters completed or scheduled of this type.
  2. Have the "Baseline" encounter completed.
  3. Schedule all of them on today's date
