Phulwari report modifications #60

These changes are to be done for all 3 implementations - JSS, JSS Singrauli, JSSCP

QA Notes -

Report - "Children Moved to Lower WFA grade", filter not showing options for month and year.
Same for report "Children Moved to Upper WFA grade"
Report - "Grade change"
1. JSS Phulwari - Remove Master catchment from catchments filter. DONE
2. JSS Singrauli - Catchmnet filter showing catchment of PHNI phulwaris. (In singrauli there are some filters with duplicate locations. keep that also in consideration) DONE
3. Everywhere Year and month filter does not have values.
4. What about the kids, where weight is not taken yet. DONE

Report - Grade Change
1. It does not show all the kids present in the phulwari at given point of time (after filter)

On hold need to discuss some issues with client.

Changes :- We need to include all child whether their encounter completed or not. so we are going to create this report on the earliestVisitDateTime. I have also informed the client that the report may show incorrect data if they did some unplanned visits
