NTLMAuthHeader should be used for DCE/RPC only #2

The common header defined in class NTLMAuthHeader in ntlm.py is actually 
exclusive to DCE/RPC, not NTLM. So if included (see r246), DCE/RPC 
authentication with NTLM1 works, but SMB with NTLM1 doesn't.
