Docs changes related TAP 1.4.1 minor release

Added Release notes section
Added WARNING in Deployment Guide
Removed configuration params:
- Version of TAP
- Version of EKS
- Linux Bastion host AMI
- Linux Bastion host's SSH port
- Sample application name
Removed Dead Code
Updated a few CFT Output params
