Mobile number needed in registration/claim screen #118

What steps will reproduce the problem?
1. Go to main website
2. Search and try to claim a username

What is the expected output? What do you see instead?
I expect to see the field "mobile number" in the required fields. But I do
not see it.

Note that this is in accordance with the screenshots sent before the
meeting on 9/3/09

Minor update needed:

When I hit submit, it shows me the summary of the info I entered, with my email 
and 
answers to questions. I think we should also show the number at this screen.

It is the listing after "Here is the info you entered."
