Update Discord domain #74

I found this issue, https://github.com/discord/discord-api-docs/issues/1585, It seems that discordapp.com is being deprecated. So I thought it would be a good idea to update the Discord domains. The other discord domains should be updated as well.

@azagaya Anything with discordapp.com should be replaced to discord.com
