regarding your note about maybe journalling isn't working correctly ... I think it is, but that the actual problem is that the automatic recovery mode mount isn't running the journal
my recollection is that when i do a manual mount in recovery mode when that happens there are logs saying that the journal is being processed
so I think the deficiency is in how recovery mode does its automatic mount of /cache when it starts up
