[META] Document upgrade paths for modules now integrated into core

We already have a "Placeholder" content type that's specifically intended to appear in searches for modules/themes/layouts to inform people that a module/theme/layout is not needed for Backdrop. I've added an additional field to this content type for Upgrade instructions.

A placeholder node should be created for every project that is not needed in Backdrop, with explicit upgrade instructions for that project. These will not only appear on the modules/themes/layouts pages, but can also be aggregated into a comprehensive list, and perhaps linked from this page
