Cleans up documentation for applicants and reviewers

This PR is big, I know. :smile:

Essentially, it does the following:

• renames folders to make things clearer
• fixes broken links
• removes the role of Moderator, which is now lumped into the Maintainer role
• consolidates documentation, which previously existed in several places, making it confusing for applicants and reviewers
• updates the processes based on our current workflow
• adds additional resources for badgers
