Applied the same process here as I did in https://github.com/bagder/http2-explained/pull/206. This one required fewer editorial decisions :-) I did keep the half-complete German translation in the TOC, but without placeholders for missing chapters since there was so much missing.

Results of my branch are perusable here.

Fixes #171.
