Merge: store list of input models used for merge #121

Use the annotation file associated with the merge model to store the list of 
input models that have been merged, and for each of them specify ID, name of 
process model and version.

The conceptual model for meta-data has been modified, so each merge model 
version is associated with process versions that have been merged.

Storing this information in annotation file rises the following issues:
- at the moment APF xschema specification does not provide any thing for that.
- as many APF could be associated with a given CPF, shouldn't this information 
be stored as meta-data only?

after a discussion with Marcello: we decided that this information has to be 
stored in the relational meta-data only.
