Create and hook up an empty properties dialog #1

When the CalDAV resource is added in Kontact, a dialog should pop up to
enter the following properties:

Text field:      URL
Text field:      Username
Password field:  Password
Checkbox field:  Remember Password

Code should be placed in config.h, config.cxx, and config.ui
