When video loading fails, offer button to disable caching #95

There is a user report showing that this helped with a weird permissions error when loading a video from the cache. Naturally if they select this option, rebuild the video player.

Implemented in 27d1a2debbb88cc6d3983f64692fef5a20ed28de. I just fallback automatically.
