Footnotes including bibliography #3

Probably some Pandoc post-processing on the Markdown output to directly create footnotes and their links.

I want to keep usual Markdown footnotes in the Markdown document (because the Markdown document should be "portable") so the transformation would happen in wp_post() via regex or something smarter.
