9 years ago After much pain and suffering, I nailed this down to the fact that the SIAM class file loads the graphicx and hyperref packages using the dvips option. Although they say not to modify their class files, I had no choice but to comment out the two RequirePackage lines in the .cls and add the packages with the pdflatex option in the preamble of the paper.
We may encounter problems later, when submitting the paper. Who knows. This class file must be from, like, the 90s.
IntegrationDomain.pdf refuses to be inserted into the page properly, despite the fact that I entered the natural width and height in the \includegraphics option (which is done when the latex process can't find the bounding box).
I also have problems with the mesh images of the red blood cells, RBC512.pdf and RBC2048.pdf, and the four-cell image: 4Cells_arrow.pdf For the time being, I hacked in some \vspace to force them to show up within the text margins, but this probably will not work with journal submission and might even fail with arXiv.
