Fix asking for name guidance

The pattern 'Asking for names' is missing guidance on avoiding latin-based terms for names, for example surname.

Products currently being built are using latin-based terms for names:

Current behaviour Names are being referred to as 'Forename' and 'Surname'

Expected behaviour Names are being referred to in plain English as 'First name' and 'Last name'

Additionally, guidance is required to:

• avoid asking for middle name unless absolutely necessary (for example need to match a government document)
• if it is asked for, make the field optional as not all people have middle names.
