Add standalone spindle-migrate utility #67

This utility will perform any one-time data migrations which aren't suitable for inclusion in the normal schema update process, because they may be long-running

Sanity check after migrating proxies into state table is required: the number of rows in the proxy table should match the number of rows in the state table.
