Scan: various restart issues #52

I start a new scan w/o exiting from scan graph first.  It has problems:
  * forgets new file name I set and other settings I wrote; asks if I want
to rewrite file
  * Gives error message when I tell it a new file name
  * Doesn't scan

More info:

I think this is reproducible:  when you accidentally forget to change the
file name in a scan (as compared to the previous scan), the program asks
you if you want to overwrite, rename, or cancel. I think the overwrite and
cancel options work, but if you choose rename, it crashes badly. It's a
nice feature which used to work, so I don't know what happened.
