User Story As a forms designer i want to receive a finalized version of the form with all the questions and type of fields associated with these questions to start building the front-end version of the form

Tasks

[x] finalize a mock-up of the forms questions, their field types, and options for response.

Acceptance Criteria When the checklist item has been made and after getting sign-off from product team, we can close this ticket

Our ED will take a look at the questionnaire next week. Once we have her feedback, I'll mark this as completed.

As soon as the form has been reworked with all revisions we can run it by Melissa Thickens and Martha Wood. The draft language will be done today
