Considering one of the goals is to encourage meaningful conversations between senior leaders & supervisors about their careers, is it better to have supervisors put their feedback without seeing senior leader's answers.

Supervisors will give more honest & thoughtful feedback, instead of saying they agree.
If the supervisors think differently than the senior leader, that will become apparent and spark an interesting conversation.
However this is not usual, and might lead to difficult or awkward situations without proper change management.
