Student- Request A Change - Declaration & Notifications

User Story As a Student, I want to be notified when I am Requesting a Change that my study dates have passed so that I can be reassessed and verified for accuracy and understand the impact to planned disbursements

Context about Current Design • Please Review the Request a Change process functions design in Request a Change Epic #266 • Each Request a Change form submitted by the student reflects a request to replace their original assessment workflow variables with new ones.

I.e. if two 'Request a Change' forms have been submitted at different times, the system does not automatically 'stack them' on top of one another - the most recent request always requests a change to the Original assessment, not the latest re-assessment. • Request a Change forms which are approved only exist for the lifespan of a single application (i.e. they don't get attached to a students profile and automatically apply to the next application) • The student's application form should NOT be changed to reflect a change that was approved via Request a Change. That form should be preserved to reflect Instead, the student's 'assessment' tab within a particular application will show the application form as it relates to a specific assessment (i.e. it captures any changes which were approved) • Each Request a Change that is approved is placed in the 'queue' with other pending changes to a student's assessment (Change to Offering details, Scholastic Standing, etc..)

Acceptance Criteria

[ ] Content - All Fields Required. After students have selected their forms, and they are inputting the individual fields, add a banner message displaying the following text: "All fields must be completed. If information has not changed since the original application was submitted, copy the original response into the field below."
[ ] Content - Study End Dates Warning. If the application requested has study end dates which are within 6 weeks of study end date: Display a Banner Message which says, "Warning! You are requesting changes to your application within 6 weeks of your study end date. You may be ineligible for changes. You can still submit your request, but this request will be subject to standard policy rules stating that students must submit all requests for changes at least six weeks before your study end date." (see photo below)
[ ] New Checkbox Acknowledgement Before a student clicks 'submit' on the Request a Change, introduce a mandatory checkbox which says: "I acknowledge that this request will not be processed until all required documentation has been provided. I recognize that StudentAid BC representatives may request additional documentation after reviewing my request, and my request cannot be actioned until I provide that documentation."
- [ ] If the student checks the box then they can submit the form.
- [ ] If they do not check the box the student cannot submit the form and Display the error message "Please sign the acknowledgement of documentation requirements above"
[ ] New Checkbox for Declaration Create section on all Request a Change Forms for Student Declaration (use the exact same one as displayed during initial app submission) - Check Box to Confirm understanding
- [ ] If Student checks the box the student may submit the Request a Change Form
- [ ] If they do not check then they cannot submit the Request a Change Form. Display Error message "Please sign the student declaration above"
- [ ] Display same warning to ministry staff
