Ministry: enable adding Notes when creating Basic BCeID account #3902

Describe the Bug As a Ministry User when I create or deny student Basic BCeID account, I am not asked to add Note to the decision. As result there is no record in a system of who, why and when made a decision. This might impact Tier1 ability to communicate this information to a student.   Expected Behaviour

  1. When Ministry User reviews Student Basic BCeID account request, they must be presented with a screen that askes them to add a decision note when approving. The Notes should be applied to the account when it is created.

  2. Notes must be stored under General Notes in Student Account.

Actual Behaviour Currently Ministry User can make a decision to approve or deny Student Basic BCeID account request without leaving Notes behind.

Steps To Reproduce Steps to reproduce the behaviour:

  1. Go to Ministry Portal
  2. Click on Accounts
  3. Choose an account and click View
  4. Click Create student account
  5. Click Create account now
