Update Add/Remove Dependant Change Request

Update Add/Remove Dependant change request page to align with changes made to the FT/PT application

Acceptance Criteria

• [ ] Make these changes for the 25-26 program year application form

• [ ] Change title of change request option from "Add/remove dependants" to "Dependant Information"

  

• [ ] Move "Are any of the dependants listed above supported by you financially but not under your sole custody?" question to the Add Dependants box.

• [ ] Revise question to: "Is the dependant listed above supported by you financially but not under your sole custody?"

• [ ] This question should be included for each individual dependant added.

• [ ] See ticket 3827 for reference

• [ ] Add the following two questions from the Dependant Information page:

  During your study period will you have unsubsidized day-care costs during class hours for children 11 years of age or under?

During your study period will you have unsubsidized day-care costs during class hours for disabled/infirm dependants 12 years of age or older?

• [ ] These questions will go beneath the "Add dependants" box in their own category named "Dependant care costs".
