Describe the task

The well authorization field (on the facility form) is currently using the default array field template. This task is about creating a new template (both default and readonly) for this field.

Acceptance Criteria

[ ] ArrayFieldTemplate and ReadOnlyArrayFieldTemplate
[ ] templates added to the themes so they display on the facility form
[ ] Vitests for both templates
