When adding new Operation Representatives, we need to display the same template as ContactForm. This will allow the user to select an existing BCIER user to populate the Operation Representative form data as a new contact.

Acceptance Criteria

• [x] Remove tempContactsSchema and use contactsSchema
• [ ] Populate the Operation Representative form with correct data when multiple new OP Rep forms are present and users select different users.
• [ ] Add tests for ^

Update the operation representative screen per this Figma Figma Please refer to the prototype for the behaviour/interactions Figma

• [ ] Remove the "is this contact a user in BCIERS". Reason for removal: given that someone is a user, they should automatically be added as a contact as well. therefore there isn't a need for having the "is this contact a user in bciers" field.
