UAT Bug-Toggle button icon not showing but functions

Describe the Bug:

I am able to toggle Yes or No on "is parent company registered in Canada?" in operator registration form, however the toggle button is not showing. The form fields change appropriately and I can add another parent company.

Steps to reproduce the behaviour:

1. Click on My Operator
2. Click on Edit
